Service Description
Your initial plan and your progress since your initial consultation will be reviewed, and additional strategies will be formulated to ensure ongoing progress in your journey for better health. Medicare rebate of $54.60 will apply if you have been referred to us by your GP on a Chronic Disease Management Plan / Team Care Arrangements. Private health insurance rebates may apply - please check with your insurer.
